Program of Hospice Care
The primary goal of hospice is to teach and assist family
and/or friends to care for the patient at home.
Hospice care is initiated by contacting the hospice office
which will send a representative to meet with the patient
and family. Referrals are usually made by the primary physician,
but can also be made by family members, friends, clergy,
or health professionals.
Caregivers will be taught how to provide any needed home
care. Although the goal of hospice is to encourage the patient
to be cared for at home through participation of the family,
if a patient is in need of acute pain and symptom control,
he may be admitted to the hospital.
The heart of the hospice program is the specially trained
volunteer who provides emotional and physical support for
both patient and family through a wide variety of services.
The volunteer may relieve the family for short periods,
run errands, baby-sit or assist in many other ways.
The volunteer gives bereavement support to the family
for at least 13 months.

professional RN visits, rehabilitative therapies, home health
aids, private duty, respite care, and assistance with community
resources to assist in everyday functions.

Coping with Loss Hospice Class
is a free 30-hour annual class and open to the public to
train caregivers to give the patient and the family the
support they need through their difficult time. These classes
well prepare our caregivers for the home care they provide.
In order to be a Direct Care volunteer, you must take this
required class taught by Sereno Joan, Executive Director
of Grundy Community Volunteer Hospice and Leslie Babson, Direct Care Hospice Volunteer. Class begins Feb 22, 2011. Call our office for more information 815-942-8525.
Hospice Lending Library
Our library contains books, audiotapes and videos on dying,
death and grieving. We encourage you to stop by and borrow
from this wonderful source of information and support. Grundy
Community Volunteer Hospice library is open during office
hours (M-F 8-4.30pm) to check out materials. Donations of
books or funds to our Lending Library are always welcome!
Hospice Volunteers
are the heart and soul of our volunteer hospice.
We are always looking for new volunteers to become active
in hospice and encourage interested volunteers to come during
office hours (M-F 8-4.30pm). The Coping With Loss training
class is a prerequisite.
Hospice Equipment Checkout
the community has been generous with donations
of a wide variety of wheelchairs, walkers, canes, commodes,
showerchairs, and various patient care items can be lent
